Utilization Review/Case Manager - Case Management
Job Summary: The Case Manager / Utilization Review / Discharge Planner provide review for appropriateness of admission and continued stay according to established criteria. Organizes the Discharge Planning/Social Services functions for acute and swing bed patients. Provide assistance to the patients and families in this institution and the community facilities and agencies that can provide economic, psychological and medical assistance. Promote preservation of patient’s physical and mental health.
Associate interacts with physicians, patients, families, directors and supervisors, peers and other associates in a professional manner in all forms of communication. Demonstrates a commitment to professional accountability and growth to maintain and expand knowledge and skills. Demonstrate professional courtesy and compassion and dependability in job performance. Works in a fast paced environment, excellent organizational skills required. Pleasant phone skills and etiquette required. Attention to detail required. Completion of chart documentation in a timely is manner required. Exhibit the values of FIRST (Friendliness, Innovation, Respect, Service and Trust).
Education Required: Bachelor’s degree, or Associate’s degree with prior experience
Graduate of an accredited RN training program
Experience Required: 3 years Case Management/Utilization Review in hospital setting preferred, including assistant swing bed and referral intake coordinator and discharger planner (acute and swing)
Licensing, Certifications and/or Registrations:
• Current RN license in the Commonwealth of Kentucky
• Basic Life Support certification (within 3-months of employment)
• Advanced Cardiac Life Support (successful completion within 6 months of employment).
