Case Manager - Case Management
Job Summary: The Swing Bed and Referral Intake Coordinator / Discharge Planner review referrals for appropriateness of admission and continued stay according to established criteria. Organizes the Discharge Planning/Social Services functions for acute and swing bed patients. Promote preservation of patient's physical and mental health by respecting patient rights and responsibilities.
Associate interacts with physicians, patients, families, directors and supervisors, peers and other associates in a professional manner in all forms of communication. Demonstrates a commitment to professional accountability and growth to maintain and expand knowledge and skills. Demonstrate professional courtesy and compassion and dependability in job performance. Works in a fast paced environment, excellent organizational skills required. Pleasant phone skills and etiquette required. Attention to detail required. Completion of chart documentation in a timely is manner required. Other duties may be assigned. Exhibit the values of FIRST (Friendliness, Innovation, Respect, Service and Trust).
Education Required: Bachelor or Masters of Social Work or Bachelor or Masters of Science in Nursing.
Experience Required: Minimum two years’ experience as CSW or LMSW with certification and hospital experience. Minimum 3 years’ experience as RN, BSN or MSN with hospital case management experience preferred.
Licensing, Certifications and/or Registrations:
Current unrestricted CSW or LMSW, RN, BSN or MSN preferred.
Advanced Cardiac Life Support (completed within orientation period).
